The candidate skips an uncertain question (e.g. Q43) in their question booklet. When answering Q44, their pencil lands on row 43 of the OMR sheet. Every subsequent question is offset by one line, destroying the entire section.

Mentora Golden Rule:Always state the question number subvocally before touching the circle: 'Question 44, Option C'. Use your left index finger as a physical anchor on the row.
Language I vs Language II Booklet Inversion30 Marks (Guaranteed Disqualification)

Candidate chose Hindi as Language I and English as Language II during registration. In the hall, they open the main booklet and solve Hindi under questions 121–150 (Language II) instead of 91–120. Optical scanner evaluates against wrong answer key.
